One or a group of people can improve healthcare through taking actions big or small to collectively improve the healthcare system. Individual acts of change, regardless of the size, can add up to the significant improvements for Ontarians, the profession and the healthcare system overall (Change Day Ontario, 2017).
The purpose of this assignment is to work together as a team to create, implement and evaluate a change initiative to address/improve an issue impacting your own nursing practice and/or nursing practice in general. The focus of the change must be linked to course objectives. The nursing practice change should be something group members are passionate about and is meaningful to the nursing profession and the healthcare system. Drawing on evidence based literature; you will identify an issue, make a pledge, then create, implement and evaluate a change. The scope of this change should be small and be completed within the time frame of this course. The initiative is something the group commits to doing by making a pledge to change (see YouTube Resources).

Proposal: due Week 5
Include a: 1) Title page; 2) Draft outline of the issue/problem, link to course objectives, proposed change project and rationale; and 3) Reference Page. Completed Team Charter should be submitted as an Appendix to the Proposal.
Written Report: due Week 12 one written report per team, 5 pages maximum (excluding Title page & References). The report should include the following:
-Issue/problem
-Aim or goal of the intended change
-Description of the change including the rationale
-Implementation strategies to support the change based on change concepts or theories presented in Week 7
-Evaluation results and analysis
-Evidence from literature to be used to support the issue/problem, change strategy, implementation strategy and evaluation. Include a minimum of 4 current references from peer-reviewed journals that have been published within the last five years and are in addition to the references provided in the course readings. Non-academic literature such as professional reports, official documents, associations websites, etc. may also be used.
