Discussion #1

Every year many nurses graduate and commence working in a clinical setting. As they start working, they start developing, growing and gaining knowledge. Patricia Benner discussed how even 30 years ago, long-term and ongoing career development was important in nursing due to the complexity and responsibility of nursing practice (Benner, 1982). She developed a model called ” From Novice To Expert” which discussed how nurses gain understanding and acquire skills through patient care over time (Benner,(1982).

I personally see myself as an advanced beginner when it comes to Dr.Benner’s continuum. I have only just recently started working as a nurse, and therefore I recognize that I still have a lot to learn (Benner, (1982) . That is why I believe I am at the point where I am an advanced beginner, which is described in the model as demonstrates acceptable performance, someone who has gained prior experience in actual situations to recognize recurring meaningful components, principles, based on experiences, begins to be formulated to guide actions (Benner, (1982). 

A scenario from my practice would be when I started recently working and had to give an antihypertensive drug via a medication pump, and I had just done it one time before( Benner,(1982). I was able to perform the task correctly because I had given this medication before but I still asked about certain steps before given the medication( Benner (1982). I was no expert at this but I was able to perform this task accurately, and many more, that like this one, with practice I will be bale to perfect.

Hypothyroidism, also known as underactive thyroid is a condition that is associated with the inability of the thyroid gland to enough thyroid hormones. The condition affects women more frequently than men. The signs and symptoms of hypothyroidism vary from person to person. The severity of the condition also affects which signs and symptoms appear and when. In some instances, the symptoms are difficult to identify (Holland, 2017). 

When assessing for hypothyroidism, a nurse should inquire about dry skin, voice changes, hair loss, constipation, fatigue, muscle cramps, cold intolerance, sleep disturbances, menstrual cycle abnormalities, weight gain, and galactorrhea. Weight gain in hypothyroidism results from decreased metabolism. Often, signs in hypothyroidism are subtle, and their detection requires a careful physical examination (Orlander, 2021). 

Areas that ae significant for physical examination include an enlarged thyroid gland, pallor and jaundice, eyebrow loss, excessive sweating, macroglossia, bradycardia, slowness of speech and dull facial expressions (Patil et al., 2021). Hands should be inspected for thyroid acropachy, onycholysis (painless detachment of the nail from the nail bed), and reddening of the palms. Peripheral tremor may also indicate overactivity of the sympathetic nervous system. Eyes, tongue and lymph nodes should also be inspected.
