Results from evaluation of the Southwest Florida Center for Children with Autism will be beneficial to the stakeholders in the organization, including funders, staff, clients and their families, and administration. The information obtained by evaluation will provide opportunity for changes to improve the organization, by highlighting the areas of success and identifying those that need improvement. Results of evaluation will assure the program is meeting the organizational goals set forth when it was established.

I would expect our stakeholders to be curious about the number of children we identified with autism through our screening program, and the number of those children that were served with the services offered at the Southwest Florida Center for Children with Autism. I would also expect them to want to know how many of those children were able to attend a general education classroom by age 6, and how many were able to graduate from high school.

Qualitative methods of evaluation, such as observation, interviews and document reviews, would be implemented to obtain our results, as it would be unethical to create a control group of individuals that would not receive the benefit of the services available at the Center.

To evaluate the program objectives, we will need to use a variety of types of evaluation. Process measures will enable us to be sure the program is implemented as it was designed and will provide ongoing information, and outcome measures will be necessary to provide information regarding the impact our program has on the children. Below, I will list the program objectives and the method of evaluation we will use for each:

Autism screenings will be held at various locations in Southwest Florida twice per month

This objective will be measured by process evaluation to determine the number of children that received a screening.

Children who fail the autism screening will receive evaluation by a multidisciplinary diagnostic team within 30 days, to determine if child has autism

This objective will also be measured by process evaluation.

Children in Southwest Florida, with autism, will receive a treatment plan within 30 days of the diagnosis of autism

This objective will also be measured by process evaluation.

Children diagnosed with autism will begin receiving individualized therapy services within 30 days of receiving a diagnosis of autism.

This objective will also be measured by process evaluation.

Children at the Southwest Florida Center for Children with Autism will be evaluated every six months and therapy plans will be revised to allow for appropriate progress.

This objective will also be measured by process evaluation.

75 % of children receiving services at the Southwest Florida Center for Children with Autism will acquire skills necessary to participate in a general education classroom, with appropriate support, by age 6.

This objective will be measured by outcome evaluation.

75% of children with autism, who receive services at the Southwest Florida Center for Children with autism will graduate high school.

This objective will be measured by outcome evaluation.

  Unit cost will be determined by the cost per unit of service, such as screening, PT, OT,

ST, behavior therapy, medical visit, evaluation, and case management. Cost effectiveness can be measured by looking at the cost per child who achieves the ultimate goals of attending a general education classroom by age 6 and graduating high school.

  The process evaluation information would be presented in tables, representing the numbers of children screened, receiving evaluations, treatment plans and services within 30 days, along with the number of children who received re-evaluations every 6 months. The unit costs and cost effectiveness would be presented in bar graph form, as would the outcome objectives. The presentations would include a narrative explanation of the findings. I realize the outcome evaluation information would take several years, but the process information along with the outcome information would give the stakeholders the information needed to determine if the program was meeting its objectives.

To determine if we are meeting the goal of having 75 % of the participants in the general education classroom by age 6, the program administrator will provide the number of children who are receiving services at the GCH-CA, who are enrolled in the general education classroom upon entry into kindergarten, report will be yearly

Program administrator will provide results from parent questionnaire, with results indicating improved relationships, reduced family stress and increased knowledge about autism, along with results about improved behavioral skills, communication skills, sensory skills, social skills and transitioning, report will be yearly

The Golisano Children’s Hospital Center for Children with Autism-Action Plan

Program administrator will work with local libraries to schedule autism screenings twice per month

Program administrator will work with marketing team to reach target audience with dates, times, locations of screenings and contact information

Medical assistant will complete intake over the phone and schedule screening appointment

Screenings will be completed by the pediatrician or neurologist, the medical assistant and autism navigator till be present

At the screening, if the child fails, he/she will be assigned an autism navigator who will assist in scheduling multidisciplinary evaluation, including medical evaluation to occur within 30 days

If child is diagnosed with autism, case manager will schedule therapies, educational services and counseling services, and assure financial coverage, to occur within 30 days

Psychologist and program administrator will develop a parent questionnaire to be administered every 6 months, to determine if family has improved relationships, decreased stress and increased knowledge about autism, along with improvement in their child regarding behavior, communication, sensory issues, and transitioning.

Case manager will administer to parents every 6 months and provide data to program administrator

Case manager will complete individualized treatment plan with parents, including therapies, education plan, and counseling services to be provided, plan will be updated every 6 months, reports will be provided to program administrator

Case manager will contact all treating therapists for 6 month updates on progress

Case manager will contact school system to obtain 6 month update and copy of IEP

Data from screenings and evaluations will be recorded by medical assistant and reported to program administrator quarterly

The finance manager will monitor the budget and will provide financial statements to the program administrator monthly

Upon graduating form the program, the finance manager will calculate the cost of services per child to achieve the ultimate goal of high school graduation, and will report to the program administrator
